Grounded in youth voice, strengthened partnerships, and data-informed action, this plan outlines clear priorities that expand our regional reach and enhance the systems that support adolescent health and well-being. Together with our communities, board, staff, and youth leaders, we are building a future where young people are empowered, heard, and positioned to thrive.

The Need
Georgia ’s teen birth rate is above the national
average, at 16.5 births per 1,000 youth.
Suicide is the second leading cause of death among youth in Georgia.
Adolescents and young adults account for 55% of all reported sexually transmitted infections (STIs) cases in Georgia.
Strategic Impact Goal
By the end of 2028, GCAPP will establish a fully operational six-region structure–Metro Atlanta, Southeast, South, West Central, East Central, and North–to implement strategic initiatives tailored to each community’s needs. Through this structure, GCAPP will impact 260,000 young people, 130,000 parents, and 50,000 youth-serving professionals, while building 60 new strategic partnerships to drive improved health outcomes for adolescents across Georgia.

Three Year Strategic Goals for 2026 – 2028

By 2028, 10% of YAC members will be actively engaged in shared decision-making and leadership within GCAPP.
By 2028, GCAPP will engage 150 YAC members, and 30% of them will represent counties outside of Metro Atlanta.
By 2028, 75% of YAC members will report improving leadership and advocacy knowledge, skills, and abilities as a result of participation.
By 2028, GCAPP will conduct 18 annual locally-based outreach engagements within each region (3 per region).
By 2028, GCAPP will train 1,200 local youth serving professionals from organizations within each region to deliver GCAPP programs and curricula.
By 2028, regional health teams will conduct 18 locally led adolescent health activities/ events (3 per region).
By 2028, GCAPP will secure 60 new strategic partners.
By 2028, GCAPP will conduct 120 health education engagements with youth-serving professionals across all six regions.
By 2028, 100% of Programs & Training staff will be able to provide training in all GCAPP’s priority programs.
By 2028, GCAPP will reach 260,000 youth, 130,000 parents, and 50,000 youth-serving professionals with programs, training, and education.
By 2028, 75% of GCAPP Programs & Training participants will report improved knowledge, skills, and attitudes/abilities.
By 2028, GCAPP will develop 5,700 digital resources & educational products.
By 2028, GCAPP will launch 54 education & awareness campaigns.
By 2028, GCAPP will reach 25,000 youth, 125,000 parents, and 40,000 youth-serving professionals through education & awareness campaigns.
